Before being seen by one of our providers, we ask that you please download the Cholla Medical Group New Patient Packet and have it ready for your first appointment. You can also check with the front desk at your group home or retirement community, if applicable, and request a new patient packet there.
These forms are fillable and can be completed online. However, you must print and sign forms before your initial visit.
