Our goal is to provide you with the best patient care and service. Here, we have addressed some of the most FAQ’s. However, if you still have questions or need further information, call our office and we will be glad to help.

Patient Forms

Before being seen by one of our providers, we ask that you please download the Cholla Medical Group New Patient Packet and have it ready for your first appointment. You can also check with the front desk at your group home or retirement community, if applicable, and request a new patient packet there.

These forms are fillable and can be completed online. However, you must print and sign forms before your initial visit. See a SAMPLE New Patient Packet here.

Please contact your pharmacy and they will contact your provider to have your medications refilled.

The provider will contact you once we have received the results and relay that information to you. Please be aware that only a provider is able to provide results to you.

Our office should notify you of results within 1-2 weeks of getting routine labs and tests done (less routine tests may take longer). If our office has not contacted you within 2 weeks of getting routine labs/tests done, call the main number to inquire about your results.

Information will only be communicated to individuals that have been approved by the patient in writing, stated as emergency contact, or a patient’s medical power of attorney.

Once a patient comes onto service with our office, they are scheduled for regular visits on a weekly, bi-weekly, or monthly basis depending on medical necessity. However, we can accommodate to see a patient outside of their regular scheduled visits if need be.

Insurance is billed however please note that you may receive a bill if your insurance decides to make a portion your own responsibility. If you are self-pay the initial visit is $250 and follow-up visits are $150.
